Please explain me about surgery "KASHEM".It is done in the throat.Please tell its benefits and side effects.


The person was operated for Goitre and by mistake thyroid glands were removed in the operation. The person is suffering now and have difficulty in breathing. Doctor says that the vocal cords are not expanding enough while breathing to allow free flow of air. By operating upon the vocal cords and placing it apart the breathing problem can be to some extent removed, as per the doctor. This operation is called "KASHEM". I require more details of this operation, its side effects, if any, etc.

I've gone thru' my books and I've not found the words KASHEM in the surgical procedures. Perhaps there's a spelling mistake?

Coming to the matter at hand, thyroid surgeries are the most common cause of vocal cord paralysis. It affects either the right and/ or left recurrent laryngeal nerve to cause either one-sided or total paralysis. Depending on which part of the vocal cord (adductor or abductor) and sides (unilateral, bilateral), the respiration may or may not be affected.

To cause breathing problems, the cords are either unilaterally paralysed (problems arise on exertion) or bilateral paralysis of abductors.

Tracheostomy is performed 1st to relieve breathing difficulties. Thereafter, there a number of ops done including, but not limited to arytenoidopexy, cordectomy, Woodmann's op (most closely linked to the procedure you had just described), nerve-muscle transposition.

Make sure the doctor explains the details of the surgery to you clearly. Learn of it's procedure, it's risk to the patient, the after-care and the sucess rate of the op & of the doctor. Be well informed. Go to only trusted surgeons in very good hospitals where they'd have the facilities to help & care for the patient. Do not hesitate to get a second opinion.
