Ventilator circuits with hme filters?


how long can a hme filter be removed from a ventilator circuit without adversely affecting a patient?

Most of the manufacturers claim that the HME filters can stay up to 48 hours on the vent circuites. However, your clinical judgement plays an important role in this matter. If you see a dirty filter already in the circuit, I am sure you are going to change it immediately. However, you need to consider the stability of patient in regard to how sensitive your patient for disconnection for the sake of changing the filter even though it takes only few seconds.
In my practice, normally all new patients will be put on to a dry circuit with a HME in it. (It is due to an cost implication- dry vent circuits are cheaper than a humidified circuits)Eventually, they will be on the humidified circuit(with a humidifier with a heating wires and temp probes) after 24- 48 hours. During this time ,we will assess them for their suitability for the extubation. If, extubation is unlikely after 48 hours, then they have to be on the humidified circuit. We use a non-HME filters with the humidified circuit just to protect the patient from getting dirt from the gas supply.
Again, it depends on their suitability. Otherwise, they will go on to stay with a HME filters on the circuit.

A vent filter takes the place of the cilia in normally-ventilating people's mouths, noses, and throats, which act to "filter" out the bad stuff in the air.

In people whose respiratory systems are already compromised, not having a filter on the vent will simply expose them to additional irritants, and this should be avoided.

Not having a filter on there for short periods of time is not going to immediately kill someone, but nor should it be missing for any longer than necessary.

I would even venture to say that a dirty filter is better than no filter.

That really depends on the pt., the HME acts as a "nose" on the vent circuit. It helps to humidify and to keep the inhaled air clean before it reaches the pt. so without an HME the pt is being delivered dry air. This will not adversely affect a person if it does not go on for to long but it is not recommended for long term vent use because the secretions will become to thick or dry and it will become difficult to suction the secretions out of the pt's airway.

Depends... They shouldn't be removed for too long especially without another form of humidity. Maybe long enough to give a med that's all. You also don't want to keep breaking the circuit... higher risk for ventilator associated pneumonia.
