Can auto-PEEP be detected on a ventilator pressure-time waveform?

Sure can. If you observe the pressure-time waveform the pressure will not return to base-line prior to the next inhalation. Typically we use a "flow-time" waveform to detect auto-peep, but it can be indirectly obtained from a pressure-time waveform. For example, if you have ventilator settings that include no set machine PEEP (a set PEEP of 0 cm H2O) then, theoretically, the pressure graph should reach zero during exhalation representing no end expiratory pressure in the lungs. If an amount of air remains in the lungs it creates a pressure that will precent the exhaled pressure from reaching zero. Physiologically every individual has intrinsic auto-PEEP; that is, air that remains in the lungs after exhalation (this is represented in PFTs by residual volume).
