Spots on my cheeks?

For about two years i have suffered from spots on my chin, i recently went on the pill and they cleared up on my chin but now i get them on my cheeks. They are large and really sore and stay for weeks when one finally goes away i am left with a scar.
I have previously tried all types of medication from my doctor for it, I live a very healthy life style as i exersise every day, eat well, drink loads of water and follow a skin care routine as I am a model and being healthy is important.
I just cant seem to get rid of them...has any one got any ideas....I am 21 and i was hoping that my teenage years were over me!
