Why does he lose consciousness after exhibiting tachycardia?


Mr. Novak is brought into the ER of a hospital suffering from cardiac arrhythmias. In the ER he begins to exhibit tachycardia and as a result loses consciousness. His anxious wife asks why he has lost consciousness. What would you tell her?

It is possible for the heart to beat fast enough that it becomes an ineffective pump. There is not a sufficient amount of blood pumped to supply oxygen to the brain and the patient loses consciousness. This is a medical emergency.

Since you already know that he has cardiac arrhythmias, my guess would be that he is in atrial fibrillation.

When he exhibits tachycardia the heart is beating too fast to fill up, and therefore, it isn't pumping OUT enough. As a result, his blood pressure drops and his brain is deprieved of blood and oxygen - then he passes out. If it's momentary it isn't dangerous, but her husband should consider seeing a specialist because arrhythmias can be dangerous. If this is common for him it just could be PSVT, caused by re-entry phenomenon or an extra pathway. Either way, it's worth exploring more.
