Can Sarcoidosis or Sjogrens syndrome ?


I have dry eye,Dry mouth,dry nose,muscle pain, Joint pain, diabeties type-II, hypothyroidism. So doctor checked ANA- positive, ds-DNA- negative, SSA&SSB-negative, HIV-negative, aTPO-highly positive.

Well,
it isn't Sjogren's sydnrome becuase your SSa & SSb are negative.
SLE is out of the question becuase the ds-DNA is negative.
Sarcoidosis does have a positive ANA, but confermation would be a SCL-70 antibody test
Polymyositis and dermomyositis also has a positive ANA and a positive Jo antibody.
the ATPO is conferming the hypothyroidism

so I would guess that it is more sarcoidosis from your symptoms. good luck

Or maybe SLE? systemic lupus erythamatosus. Could be sjogren's also, or another autoimmune condition. But i'm not really thinking it's sarcoidosis.

Ok I did a little bit more reading on this problem for you. I didn't read the aTPO part before, and it does indicate an autoimmune thyroid disease - in particular since you say you're hypothyroid, Hashimoto's thyroiditis. But maybe you already had this condition (as it's not entirely uncommon) and it's occurring concurrently with another autoimmune condition.

My rheumatology professor always said to look for uncommon presentations of common illnesses, rather than common presentations of uncommon illnesses. So yes, ds-DNA is negative, but it is in 10% of SLE. Though anti-RO and anti-La are negative, only 40-60% sjogren's is pos for them. A better test to rule out sjogren's is rheumatoid factor. I think you should also get some ANCA antibodies done too to look for vasculitis causes.

If sarcoidosis is still suspected by your doctor, s/he should do a chest xray. But I thought with sarcoidosis would be more respiratory symptoms like cough/chest pain and maybe a fever?
