Radiotherapy Report?

I had stomach (cardia) cancer and I undergone a total gastrectomy then I took 25 sets of radiotherapy with a mask on chest and abdomen......... finally the radiotherapy doctor wrote his report as follows (the patient received a PORT to the stomach "confirmed planning" 4500 cly/ 25 fr/ sw)..... so what does this mean?

Some of this answer is educated guesswork on my part but I know a little about it since I am a doctor and my wife recently had radiotherapy for breast cancer

I would guess that a "PORT" is probably a "prescription of Radiotherapy" (RT is always short for Radiotherapy in this instance)

the numbers and letters will be the specific dose and type of radiation frequency used eg 25fr will be 25 fractions or doses. Radiation specialists refer to each dose as a fraction
