VQ scan and pulmonary embolism?


A VQ scan for a pulmonary embolism showed initial probability in the right lower lobe but the patient had an infiltrate at the same time. What is an infiltrate and how would it affect this test? Where would an IVC filter be placed? Thanks

An infiltrate is consistent with pneumonia and makes reading the scan difficult, possibly creating a false positive result. An IVC filter (or Greenfield filter) is placed in the Inferior Vena Cava, or the main vessel that takes blood back to the heart and lungs to be oxygenated. The idea is to catch any thromboemboli (clots) before they reach the lung and get lodged in the smaller vessels there, causing focal lung infarction, or death, and possible sudden death of the patient.

I have had them before but they never implanted a filter! Not sure I like the idea when I'm throwing clots! Generally, they stick me in ICU with a heparin drip, coumadin, venagrams, oxygen levels etc. It's like the best thing if you're into torture because you'll get plenty! I know they sometimes surgically remove the clot too if it's life threatening! I like that term- "life threatening"! They are all life threatening! Dangerous business! I also get them in my legs and arms too and they are very painful no matter where they are!
