Writers or Physicians--I Need a Disease?

I need a disease, similar to phenylketonuria, requiring a drug to keep a child alive from a young age. I'm trying to determine if I should invent this disease (for a fantasy story) or if there is an existing disease that I could use. How could such a disease be diagnosed at a late 18th century tech level? (I can use magic, if necessary.) Are there any such diseases which don't cause or aren't concommittant with mental retardation? Thank you for your help.

I don't think it was possible to diagnose many fancy diseases in the 18th century, like today.
Think of magic, one child needed a particular herb only found on Himalayas to keep him/her alive.
Good luck with your story!
