Does a patient have any rights in decisions about their healthcare anymore?

Do I have the right to refuse novocaine for dental work? To refuse an epidural during labor? To refuse sedation during an endoscopy or colonoscopy?
It seems to me that patient's have no rights anymore and I find that sickening. I refuse drugs all the time and the doctor/dentist always try to talk me into being sedated, knocked out, drugged, twilight sleep, etc. Why do they do this? If a person has an extremely high pain tolerance and DOESN'T want to be drugged, why do they keep you there another hour trying to talk you into it? Is it because they can bill the insurance company more? I'm sure it all boils down to money, and that's why the healtcare industry is getting richer, and us poor slobs that have to pay for it, getting poorer.
I'd love some feedback on this, especially from those that agree with me on this point. Thank you!!

Well, it could be that they are afraid that u do not have the pain tolerance u say u have.. And to do the procedure right and not to keep you in so much pain so long they try to give you something that will help. If u do not want to take the meds that will sedate u ask for something less potent. Like Vicodin for pain or maybe just lidocaine to numb certain areas. I have alot of experience knowing that people are not comfortable with being sedated due to all the stories. So if u don't trust the person you are going to ... Get someone u DO!!!!

If you're over 18 then you can tell the doctor: No, I don't want it. that's final.

they'll push for it but unless you're incapacitate or they can't complete the procedure without you being sedated then I don't think they can require you to have the drugs.
