Diagnose this patient: school project?


The patient is a middle aged man who noticed a lump on his right forearm, which is solid, firm and red, with a slight heat. When he first noticed it, the lump was the size of a pea, with no breaks in the skin at all. Four days later on December 16th the patient went to his doctor because the lump had grown to the size of a golf ball. At this time a blood culture grew nothing and the patient was given a shot of keflix and sent home with Septra (SXT) Bactrim. Three days later the patient went to the emergency room because the bump was the size of a softball, he was feverish and delusional with a blood sugar well over 500. By this time the bump hurt very badly (pain scale 1-10, 8 or 9. 10 being unconscious); his whole arm was red with multiple red lines going up. Blood culture was done and the results where moderate Gram positive cocci and many Polymorphic nuclear cells. what does he have?

The patients sounds like he had an abscess that was inadequately treated. The abscess probably led to the patient becoming septic. The infection can cause the blood sugar to go up and also accounts for your blood culture.
