What is right hilar focal FDG uptake with max SUV 2.7 in a pet ct scan report mean?


I underwent left mastectomy and immediate reconstruction through DIEP flap tram last year.My nodes were negative.ER-Negative, PR-Negative.Had 6 cycles of chemotherapy and radiation.Now the PET CT SCAN report says:
There is intense FDG uptake in the chest wall skin which is thickened with max SUV 4.6.
There are two discrete centimeter sized axillary nodes on the right side with FDG uptake (Max SUV 3).
There is a right hilar focal FDG uptake with max SUV 2.7

Generally we take SUV of 2.5 as the appropriate cut off point for differentiating between benign and malignant lesions.

However, we need to exercise caution because:
1. There are always grey areas in medicine. There is no such thing as definitive.
2. Infection and inflammation also cause increase in SUV
3. Different PET centres use different methods of arriving at the SUV.
4. We cannot interpret SUV in isolation. We need to know the proper context of the patient and the disease.

Thus to answer your question, SUV of 2.7 may or may not be significant. There are a number of lymph nodes in the hilar region. The question here has these nodes become cancerous? I suggest talk to your oncologist for more definitive answer.
