Nursing diagnosis for a patient w/cellulitis?


pt. has a new ostomy appliance for wound drainage (this is a temporary solution), is in chronic pain, on temp. bed rest, diabetic

Naturally, most of these will be based on your individul assessment of the patient. But here are a few.

Altered level of consciousness r/t pain and medications used for pain management.

Risk for infection r/t invasive procedure and wound to skin.

Impaired skin integrity related to inactivity, drainage, and underlying physical condition secondary to ostomy, bedrest and diabetes mellitus.

Risk for caloric imbalance: more than metabolic needs related to bed rest and diabetes mellitus.

Disturbed body image related to ostomy appliance and draining wounds.

Risk for loneliness r/t prolonged bedrest

Deficient knowledge of self care r/t ostomy appliance

Readiness for enhanced knowledge of self care r/t ostomy appliance
