Physical, Occupational, and Speech Therapy ICD-9 Codes....?

I work for a Physical Rehab Clinic in Miami, and I use ICD-9 codes very often. I want to create a "shortcut" sheet for my coworkers and I, but I can't seem to find one already made online, or a format to follow.
I have a couple ideas, but very little time to create a master pieceeeeee.

Give me sitessss, or your own work to start off with.
Thankssssssss

Just take note of the diagnosis you see come through the clinic. compile a list of the frequently used diagnoses then look up the icd-9 codes and list them in alphabetical, then again in numerical order. You'll end up with two separate lists. Keep in mind that this may take awhile for you to complete. Don't expect it to be an overnight project. Take the next two or three weeks to determine the diagnoses that are used most often in your setting. I have shortcut sheets for icd-9 codes but mine are specific to my setting which is geriatrics. good luck
