But you could ask the patient to smile (this should be even), speak a simple sentence (coherently), raise both arms or poke out their tongue (it should be straight). If you or someone else has problems with these simple tasks then call the emergency services.

I believe you are talking about TIA's (trans ischmic attacks). I can not see how this because when you are asleep your blood pressure falls, as this is the main cause of stokes or TIA's.

High blood pressure forces blood through weak vessels which then burst, leaking out into the surrounding tissues, hence a stroke.
