What does medicare part A, part B and part D cover?

I am a 43 year old female. I am disabled due to two back surgeries and have not been able to return to work after 3 years. I am waiting to recieve conformation about being granted my SSI disability. I am trying to figure out which form of medicare I should sign up for. I have had no medical coverage of any kind due to not being able to afford it straight out of pocket. I want to choose the right one for me. I do take five different medications on a daily basis. Most of which I can get on the Wal-mart $4 generic plan. I am still under doctors care for my back and take muscule relaxers that are not covered under the Wal-mart plan. I hope that this will help you answer my question.
