When a woman has a mastectomy, is the nipple removed also?

Yes. If a woman has a mastectomy they remove all breast tissue. My aunt had a double mastectomy and they removed all breast tissue including the nipple to be as sure as possible that they got all the cancer. There are ways that a nipple can be constructed out of other tissue, though.

Masactomy operation involves removal of the entire breast either the left or right which has cancer cells in it. So the nipple also gets removed.

I think it's possible to get reconstructive surgery, and have an areola and nipple tattooed on/repigmented. As always, I'm sure there are risks involved, but it's something to research if you're interested.

Yes, almost always, in order to remove all breast tissue..

Sometimes, a surgeon does a 'skin-sparing' mastectomy so that reconstruction 鈥揺ither done at the time of surgery or at a later date 鈥?will look more natural and realistic.

There have been some skin-sparing mastectomies done where the nipple too is saved; for such an operation the patient would have to be considered low-risk 鈥?for example with a small tumour, a long way from the nipple.

Most times, yes, the nipple is removed. Sometimes, a nipple and skin sparing mastectomy is performed but even then, the nipple is usually removed and scraped. A skin sparing mastectomy is the norm nowadays unless a woman is positive that she doesn't want any reconstruction to make a new breast mound.
When I had my double mastectomy, my surgeon discussed saving and scraping the nipple on one of my breasts since that breast contained precancer and not an active tumor. She ultimately decided against it with my blessing.

Based on my own experience-yes.
