What is a managed care? and how it is different from HMO and PPO?

Are there more organizations in health care besides these?
please answer in detail if you can.
thanks in advance.

hmo is where you choose a primary provider or doctor, this doctor then refers you to other specialist doctors if needed. The primary provider doctor gets a monthly payment because you chose that doctor (like 20 bucks or something) this plan is supposed to be less expensive

ppo is where you can choose basically whatever doctor you want, this plan is supposed to be more expensive because the insurance companies don't negotiate as much with them, and you might choose to see as many doctors as you like without going through a primary provider doctor
