I need to download form stating "no artificial life support for me" to give my kids. Where?


A legal form that states: should I be unable to speak, and/or comatose, I do not want to be hooked up to machines to be kept alive. Where can I find a copy of the form on the internet?

This is such an important thing for all of us to do. You need to do a Yahoo or Google search for "advance health care directive" and your state. They are inforcable. Make sure you talk to your family, give them a copy and have your doctor put a copy in your medical chart.

I don't think a downloaded form would be legal, at least if someone might contest it, but it's called a "Declaration of Intent and Durable Power of Attorney for Medical Decisions."

My parents did this last year and everyone in the family has to have a copy with everyone's signature, so there was a lot of mailing forms to each other.

It's pretty hefty (50 pages?) so I don't know if it's something you can just download and sign.
