I have a lower back injury. Left leg weakness, numbness, sharp needles if i move a certain way.?

I've had xrays showed a bit of arthritis, Dr. treating me for muscle strain. I know this is not that. Had several this is much different. I can bend but not for long before I feel a slight grind or (joint pop) in my back and hips. Im in PT they also feel its bone not muscle. MRI didnt show much

Work comp Dr. didnt get into specifics of the MRI. I generally have more "compressive" feeling standing than sitting. Night time is the worst pain and mobility wise.

What is that MRI didn't show "much?" I'd be happy to try to answer this question for you...but I would like to know specifically what the MRI demonstrated.

Other things to consider include nerve compression due to muscular or soft tissue restrictions outside the spine. This may include piriformis syndrome, peripheral nerve entrapment, sacroiliac dysfunction.

Let me know more specifically what the MRI demonstrates and we can discuss if it is relative to your symptoms. Sometimes what appears minor on an MRI can be a source of significant symptoms...especially because MRI are taken with us lying down and our disc may be doing something completing different while standing or moving.

Revised: It's too bad they didn't give you specifics. It makes me wonder if you have bone spurs which could be creating a stenotic type condition by which the nerves become compressed between the narrowed foramen.

Sacroiliac dysfunction is certainly a consideration. Yet, I find this condition really primarily exists in women who've had a recent childbirth. Than again, your history of sacral ligament sprain could lead to and sacroiliac dysfunction which could be causing displacement of this joint.

Either way, it is possible you could be experiencing intermittent nerve compression. If it were me, I'd insist on seeing a neurosurgeon (not that surgery is necessary indicated, but he/she could at least rule out stenosis and/or bone spurring).

Revision: Your PT is right...since the fluid in the middle of the disc displaces easily, you may not have any significant findings on an MRI. Until they can do MRIs sitting or standing, what is happening to you may never be fully understood and that's why your symptom response is very important to the therapist. You may very well be experiencing an intermittent disc derangement
