Knee injury after 6 years?

I injured my knees 6 years ago after changing my cycling shoe cleats. I'd worn the same cleats for 10 years, then when I changed them the new ones must have been in a slightly different position and after cycling for 3 months with the new cleats my knees were really sore. I rested for a long time, went to a biokineticist, did strengthening exercises religiously and it didn't improve. I ended up going to a sports physician, physio, orthopedic surgen and podiatrist over the years. All gave the same diagnosis (runner's knee) and same advice - exercise.
The exercises don't help at all. I rested for three years completely and this weekend I got on my bike for the first time, did a 20km ride and now my knees are sore again. Has anyone battled with this injury and managed to conquer it? If so, how, I'd love to know.

you don't give your age; it may help.
