Inborn defects, overweight, transmittable diseases, infertility, anxieties, fear, stress, removal of ovary/ies, fallopian tube pregnancy or blockages, cancer of the womb, deformaties, and so on.

age is a big factor. the older you are the harder it is to concieve since your producing worse quality eggs. weight and diet can be a reason. some illnesses you have can cause infedelity or stds if not treated like gonaria. also how good your partners sperm is makes a big difference. many other ones out there that icant think of right now but hope this helped too

Cushing's Disease, PCOS, stress, blocked tubes, hypo and hyperthyroid. I suggest getting a full pituitary panel, seeing a reproductive Endocrinologist, and a MRI of your pituitary.
