Incontinence assessment tool?


can anyone help me to find a good assessment toool to recommend to my nursing home to use to assess clients on admission. we do have one but it's a bit dated.
Thanks in advance. have done a web search with little luck.

I saw a couple of places you might want to look at. First run a search for "bowel and bladder assessment". Go to website #6. MED PASS-HOME. They have B&B forms you can print out. I also looked at another but it only covered bladder assessment. Run a search for "incontinence assessment" and go to website #1. Urinary Incontinence Assessment (PDF). It's a PDF file.
