Help! i have impotence! i think?

it started when i was young. i can get a huge erection when masterbating, even when a woman is masterbating me, but right when im about to "put it in", it shrinks down to the point of being flaccid . i have no idea why. i have managed to have sex before, by having the woman on top of me and hovering slightly over my penis while masterbating it. that way, when its fully erect all she has to do is go down on it....and if your wondering...YES .... if my penis comes out of her for to long ( 5 sec ) it will go flaccid .....am i the only one that gets it this way? what can i do to fix it?.......
sidenote: i feel NO pain down there, during erections and i never injured myself down there.

That's not a physical problem. If it were, you wouldn't be able to get hard at all.

It's a psych problem. So, you should probably go get some sort of sexual dysfunction counselling.
