Immunization records...? |
I know that in Wisconsin, you can obtain immunization records online, but does anyone know how to get them from Indiana? My friend is trying to become certified to watch my kids as a state-registered provider, but I believe she has to have her records to do so. She didn't move to Wisconsin until about jr. high age, and there's no records in Wisconsin for any immunizations, so we're trying to find a way to get these records. Any help is appreciated. Thanks in advance :) Contact the clinic or health unit that she received her immunizations. She doesn't have a card with them written on? Here in Louisiana we can pull up a child's shot record online if they got their shots here, ask a clinic in Indiana if they can pull them up. If all else fails she needsto have her vaccines administered again. It won't hurt her. She should be able to contact the doctor who did the immunizations and get the records from him. If she can't, she might just have to have them redone. |
