If you lost your Immunization card, how can you get a copy? |
I need a copy of my immunization card for a program at my college. But my parents dont seem to have it or have lost it. Where can you go to get a copy of it? Your doctor's office or the place (clinic, board of health) where you received your immunizations. You might also contact the board of health to see if they keep records. Check at you local health department or where ever your parents got your shots at. Most people take there children to the health department so I would check there first. |

You can't. Those waivers need to be backed. Call your high school - you needed the MMR to get into kindergarten, and they keep your records on file for something like 10 years. They should...
