Can't find my immunization records? Need them for a job. What to do? |
Okay so I get a new job today. I was aware that I would have to do a background check but I had no idea I would need a physical too. In order for me to get my drug testing and physical I need to bring my immunization records...One problem, I can't locate them anywhere. I'm currently uninsured would my last health insurance provider have these records? yes, your last provider should have your medical records, just call your previous doctor's office and ask them to fax you a copy of your shot record. Yes You probably need to go where you've had your immunizations, whether it be the Health Dept or a dr. office, they'll have the records. Your local health department may have records. Your insurance company will have records if they paid for the shots. Your doctor should also have the information in your medical chart. |
