How can someone look up their immunization records?

Lived in South Australia until 14 then moved to Queensland. Is there a record kept of immunization and if so how can it be accessed?

Its the national immunisation register, physically located in Perth but accessible via the web.

Inthe USA the health Dept has these records Im quessing Australia has a health dept or its equilent
