For yourself or your kids. If you don't have one, tell your doctor next time you are in and ask for one. If they have records of your old immunizations (or your children's) they can transcribe them onto the new card but you may have to bring a photocopy of your old records and bring it with the yellow card.

If you are still in school, you can get a copy from them.
