Where can I get a copy of my immunization record?

I need proof my immunizations are current to apply to graduate school. I don't think my pediatrician is even practicing anymore, I'm 32. I have a hep B titre and am getting TB test, but where can I get proof of my MMR, polio, etc.? Can titres be drawn for these as well?

My daughter just did this for her school. She called the health clinic and asked, they just mailed it. Im not sure though how long they keep them. She's 18. But give them a call its worth a shot, they may be able to tell you where you can get them if they dont have them.

You have to contact your pediatrician for this or go back to the clinic or the hospital where you got these vaccines because these are only given for pedia and let's hope they still have the old records. Your mom must know this. Ask her laos if she can remember the days you had these vaccines. God bless

I'm sure of titres for MMR, Polio. TB Mantoux test should be done every year especially if you work in healthcare. The best advise would be to check with all of your past Drs. and compile your results

Local Health Department...or the Dr.'s office where you received your shots.. it should be in your medical records somewhere...good luck

Your parents should have one. If they do not then you will have to go to the hospitals or clinics where you were vaccinated and ask for that info.

try the county health department where you were raised. they may have the records, or know of a way to get them.

A vaccinated person is MORE likely to get a disease than a non-vaccinated person. The whole theory of vaccination is flawed. It causes a weakening of the immune system thus making those who are innoculated more susceptible to disease.
