I lost my immunization record. I need it for my new job. Do I have to get it again. Where can I get a copy?


I'm 32 so its been a long time since I got it...

Ask your PCP to run a titer series for you. The results will prove you've had your immunizations and are accepted by most employers in lieu of an original record.

The titers simply measure the amount of antibodies in your system for the diseases, usually measles, mumps, and rubella. Please contact your primary care provider. He or she will know exactly what you're asking for and will order the lab test. It's a very simple and quick procedure. Report It

Your Doctors office. They should be able to give you a copy.

Call your gp, he or she should have a copy of it on file.

If you've have had an immunization or have been seen by your doctor within the past 3 years, that doctor's office will still have your chart on file with your record in it. If it's been longer than that but less than 7 years, your chart will have been archived in some distant storage facility and it will take time for record retrieval. If it's been longer than 7 years, it's history.

If your new employer insists on an original, you may have to start over. If they will accept a faxed copy, your previous employer may be able to help you out if this information was required for that job as well.

Health Department.

Check with your former employer and see if they have a copy, and if not, maybe your high school will still have it.
