How do I track down my immunization record?


I am living in Georgia, but I was born in Indiana. I need these records to continue on through nursing school. How do I get these? I am 31 so do they even still exist? Thanks for your help.

Ask your mom to give them to you. She was required to maintain them for you. If she gave them to you and or you lost them you will need to be re-vaccinated for some and for some you can get a Titer on your blood to see if you need some. It will prove to the school that you had the vaccinations. You should need a Tetanus (Tdap), TB test, 4 Polio, 2 MMR and 3 Hepatitis B. I would also have 2 Hepatitis A, the 3 HPV, and a Meningitis. Now don't you want to find that shot record?

start from square one and get updates on all of your vaccinations -you'll need them anyway. keep a record of these for the future.

ask your mom whom your Doctor was and he may help from there or u can ask your current Doctor or call the hospital and they can give you a number of a place that keep the records of all immunizations

you might try contacting the board of health. in the state that you were vaccinated in.

I think retracing what hospital you were born and maybe checking with hospital records to see whom your primary physician was might be a start. I also think maybe the elementary school you went too might keep records...after all you had to prove to the school you had your immunizations. If your parents can tell you where they doctored when you were young, you can also check with that office or clinic and get a copy of your records. Hope this helps!!
