Can someone out there who is a nurse/nursing student help me in obtaining a SOAPIE format for a deformed penis

I am a nursing student who needs some help. I know that the SOAPIE format is outdated, but for my school/teacher he wants me to do a SOAPIE format on this patient that I have. He also has problems with urinating, low self-esteem, cannibus dependant, diabetic and hypotension. I need some assistance if someone wouldn't mind helping me with. Thank You So Much.
