Most commonly idiopathic i.e. unknown, occasionally familial mutations in cholesterol receptors or apoproteins which carry lipid in the blood. A role for diet, glucose intolerance and diabetes, other metabolic hormones has been proposed and seems likely. In summary, multifactorial.
