Has anyone ever notice that one calf and ankle are smaller then the other?

My son has been walking with a peculiar gate for about 8 months. I broght him to the dr for a check up about it. I ask them to take an xray of the hip in case of hip displasia and that came back normal. Now it has been a couple of months and I noticed that he can not stand on his heel on the affected side.
I don't believe there has been any injury and he has never complained of any pain, but one ankle is almost an inch thinner then the leg with the normal/forward facing foot and his calf is even slimmer. What could be wrong?

Never seen it before. Sorry.
