Do all HMO cover the cost for the Tubial Ligation procedure?

I have never seen any HMO deny sterilization procedures. However, some hospitals may refuse to perform it (mostly Catholic hospitals) unless there is medical necessity. When I had my tubal, I had to go through a "Sterilization Committee" at my local Catholic hospital.

Yes they do. I have never seen one that doesn't!
