Can anyone give me a hypothetical situation where your HIPAA regulations have been violated?

Can anyone give me a hypothetical situation where your HIPAA regulations have been violated?

This is one example:

Dr. Smith is approached by a research firm, asking him to give information regarding his patients who have cancer. Instead of giving him a statistical rundown like "I have 30 cancer patients, 15 whom are on chemotherapy and 15 who are scheduled for surgery" the doctor tells the research firm the names, addresses and medical status of the individual patients themselves.

HIPPA is violated when your personal health information is disclosed in situations where your life is not in danger. If you are a dependent child, there are limitations. If you are an adult, unless you threaten harm to yourself or someone else (in cases of mental issues) HIPPA rules allow notification to proper authorities. If you have specific questions, ask your doctor or pharmacist.

I can give you a real one....Yes, it was sad but HIPAA laws are HIPAA laws and they were violated. I received a phone call...."Is this Mrs. _____" ? Of course it was me so I said yes. They then said well this is so and so from Dr. _____ office and we are sorry to inform you but we just received a call from your cousin and your Uncle is dead and your cousin is alone at the house and someone needs to either be notified to get over there with him or you need to get over there to be with him A.S.A.P. They proceeded to apologize for calling me and explained that they got my number from my records and then got the information from one of the nurses that this was my Uncle and that my cousin was a patient there too.
Under these circumstances I did not do anything but the laws should still have not been violated. NO exchange of information is to be given out. There was no way of them knowing whether or not that was really my cousin or anything else.
