Urgency. Please help me!?


I got a difficult case and I dont know which disease the patient got. Please help! This is a 25 year old man admitted for perforation of the colon. He already admitted 3 weeks ago for numbness and weakness of his arms and legs , accompanies light abdominal pain. He is diagnosed Guillain Barre syndrome
and managed to discharge 5 days after though symptoms not resolved. After several days his abdominal pain got worse and he come to another institution which refer him to my hospital because of a surgery emergency. Operation reveals perforation and necrosis of the descending and traverse colon due to mescenteric ischemia . Further exam show digital necrosis of his 2 feet and muscle weakness of his 4 limbs.
routine lab tests : nothing specific. I think of a disease that may cause multiple ischemia. Which investigation we should do?

I guess the first thing to do is to consider if the ischemia is related to the Guillain-Barre. With Guillain-Barre, there can be cardiovascular involvement, including hypotension and arrhythmias. You might also want to talk to a neurologist and see if his picture is still consistent with Guillain-Barre. Maybe there is another diagnosis that can account for both the weakness and the ischemia, like some kind of vasculitis. The sources below may help you to sort it all out. Good luck.
