Holter Monitoring?


Two years ago, my echocardiography yield the following conclusion:

-normal left ventricular dimension with adequate contractility and systolic function;
-dilated left atrium
-there is a diastolic dysfunctions, stage III
-normal pulmonary artery pressure

Recently, I seek for a second opinion from other cardiologist and he instituted holter monitoring, which has the following result:

- sinus rhythm
-no atrioventricular conduction defect\ no intraventricular conduction defect
-rare supraventricular ectopic activities with one PAC singlet and a triplet PAC's
-no PVC
-no ST-T abnormality.

Does these mean that my heart has improved of healthier now?

what is the meaning of all these result? My doctor said that there is nothing to worry about, and has not prescribed any medication, except that I have to avoid fatty and salty foods.

Still, I am bothered by breathing discomfort mostly after midnight. Please help.

The problem with taking two different studies is that its like apples and oranges.

An echo - sound waves bounced off of the heart showing dynamic contractility, valve function, the size of structures, etc. In essence, the plumbing.

A holter monitor - a 24 or 48 hour monitor that records the rate and rhythm of your heart. In essence the electricity.

So you have a report from the plumber from two year ago and one from the electrician now, and you are asking the electrician if the plumbing got better? Hmm, you can't really know.

But the electricity is fine.

The problem with diastolic dysfunction is that there is no particular cure - or even medications to improve the problem. The left ventricle does not expand well enough to fill with blood. Therefore even though your heart contracts fine, it doesn't fill very well - and if you are still pumping less volume than if your heart did expand normally.

The keys to treatment are keeping your heart rate slowed (about 60 bpm). And ideally controlling blood pressure - between 110 to 120 systolic.

I disagree with the idea that your mitral valve is malfunctioning - or the report certainly would have mentioned it. I suspect that the atria is enlarged because it is having to pump blood into a stiff ventricle (the aforementioned diastolic dysfuntion).

I think you 've some problem with mitral valve.May be mitral stenosis. It causes hypertrophy ( Increased mass or you can say dilated on X- ray) of the left atrium and this leads to pulmonary hypertension and breathing problems. Please do you Echocardiography and ECG.
