Living wills and advance directive?

A co-worker brought us a Living Will and Heath Care proxy that they used at the hospital (she works pt with me) but it is not exact enough. I found some other info online (that you had to pay to have the wiull drawn up) if I printing that info out can i attach to the advance directice from the hospitalm have my witnesses sign it and will it be binding?

ALSO, do my witnesses have to be any of the people name as agent. I am not sure I want to tell my primary agent that I picked him or why.

You can write your own will out and have it be legal and binding, just have it stamped by a notary, and witnessed also. And no your witness does not have to be anybody in the will.
