Tweety Bi gave the most accurate and comprehensive answer to your question. I'd like to add that bedridden elderly and post-op patients in the hospital are still probably one of the most at-risk people. Hospitals and nursing staff are most aware of this complication for the last several decades and this the reason that in addition to checking the patients, they now get post-op patients and even patients having heart attacks are up and sitting, legs dangling over the bed and even walking the first day.

As far as survivors of DVT, I am one. At the age of 24, I had a DVT which resulted in a pulmonary embolus. I had no symptoms until I had the embolus. I was hospitalized for a week on IV heparin therapy, followed by oral med (Coumadin). My primary care physician and hematologist felt that my condition was the result of taking birth control pills and smoking. The BCP pills had a higher estrogen content.

Since DVTs often occur in leg veins, the symptoms are usually swelling in the calf or behind the knee, redness, warmth and tenderness. Those most vulnerable are the immobile. Immobility can occur due to the effects of many disorders and status post conditions. When immobile patients are hospitalized, routine DVT checks are made by nurses. If the patient is verbal or non-verbal but alert, dorsiflexion of the feet is a manuver used to detect possible DVTs. A positive dorsiflexion indicates a need for a Doppler ultrasound to confirm or deny.

Ambulatory patients may complain of leg pain on ambulation. Again, this requires the same assessment methods.

Even airline passengers on long flights are vulnerable due to altitude and prolonged immobility and should get up and walk around at intervals.

Precautions include ambulation, ROM, TED hose, sequential compression and routine assessment for those at risk for DVT.

For those at risk, preventive treatment (prohylaxis) includes heparin and Coumadin therapy. Often heparin injection therapy is administered first and eventually switched over to oral Coumadin, which can also be given by feeding tube.

Treatment includes bed rest, thrombolytics and anticoagulant therapy.
