Where can I get Affidavit for a parent to give another adult authorization to geta minor child medical care?

I have a nephew staying for 8 months at my home while parent is in another city.In case of illness I need to get medical attention promptly for him.

The parent can contact childs Dr. and go in to Dr. office and sign release form and Dr. can send it to clinic/ hospital where you will be taking child.The clinic in your area you wish to use will have a social worker to assist you if you encounter difficulty.But papers signed by parents has to be notorized verrifying they are true parent of said child.

You can get a medical power of Attorney from the parents. This will work in any medical situation. You can go to this website to either copy theirs or use theirs as an example to draw up your own.

The main thing to remember is to have the parents signature notarized. Most banks will do this for free for their members.
