What does ASHD with CABG stand for?

What does ASHD WITH CABG stand for? I know i have Mitral Valve Replacement,Hypentension and COPD.But what is
ASHD with CABG stand for and what does it mean?

Arteriosclerotic heart disease
Coronary artery bypass graft

i'm not sure about ASHD but CABG is: Coronary Artery Bypass Graft (a heart bypass in other words)

ASHD = arteriosclerotic heart disease
