Conditions known to be associated with congestive heart failure include....?


1). coronary atherosclerosis
2). diastolic pressure chronically elevated
3). successive sublethal infarcts
4). decompensated heart

All of the above.

Edit: Coronary atherosclerosis is only indirectly associated in that it disposes to infarcts.

If you need a best answer, #4 is actually just another way of saying congestive heart failure. All of the other conditions can be present without actually having congestive failure but often lead to congestive failure.
