In 22 weeks pregnancy all Ig G titres of torch test are raised.As a doctor how do I manage the case ?

Torch is agroup of Toxoplasma gondii,Rubella,Cytomegalo virus&herpes simplex virus infections,,which if infect during early pregnancy may lead to fetal malformations, growth retardation or fetal death. Because of cost factor many a times only few selective (IgM/IgG) titres are tested. When IgG s are raised should I ask for IgMs or repeat IgGs ? Also the patient had two abortions before. they are keen on continuing this pregnancy. tellme more about management of such cases

I'm no expert on obstetrics, but IgG antibodies to each of the TORCH organisms would indicate past infection (or immunization in the case of rubella) to each of these, and IgG antibodies can be expected to persist lifelong. All the TORCH organisms are common, it wouldn't be surprising for many women to have had the full hand at some time in the past. I would be doing IgM antibodies to check whether any of these infections are current or recent.

The pathologist at the lab who did the test can also advise: I find them a fount of useful help in interpreting test results.

If the IgG titres are higher than titres done earlier in the pregnancy, I would contact the pathologist for an opinion on whether such a rising titre is significant.
